「本地特色旅遊鼓勵計劃」(第二階段)申請表

每份申請表只能包含一個「本地特色主題旅遊」旅行團的申請。

Each submission can only include an application for one "CTIS Thematic Travel" group tour. 

(Applicaiton period: to 30 April 2025)

注意事項:

• 以下提交的各項行程資料必須與已登記的行程資料內容相符 (例如: 旅遊地點名稱、旅遊體驗活動等)。
• 照片要求 - 導遊手持旅行團旅行社旗幟或印有旅行社名稱的紙牌於到訪三個已登記之特色主題旅遊行程的地點內拍攝照片各一張，照片須清晰顯示導遊的面貌。

Please note:

• The itinerary information submitted below must be consistent with the registered itinerary (e.g., designated sites of thematic travel, travel experience activities, etc.)
• Photographs must be taken inside 3 of the pre-registered sites visited during the itinerary (one photo at each site), with the tour guide's face visible, holding a flag or signage with the travel agent name.
  
  

聲明甲部 Declaration (Part A)

1. 申請人確認已獲旅行代理商的充分授權代表其提交此申請表，申請人聲明已仔細閱讀「本地特色旅遊鼓勵計劃(第二階段)」: 申請指引以及本網上申請表格的所有部分，並完全明白其中內容及同意遵守當中的所有條款及細則。The applicant hereby confirms that he/she has been fully authorised by the travel agent to make this application for and on behalf of the travel agent. The applicant declares that he/she has carefully read all sections of the Characteristic Local Tourism Incentive Scheme (Phase 2): Guide to Application and this online Application Form and fully understands the contents thereof and agrees to comply with all the terms and conditions of the above documents.

2. 申請人明白，香港旅遊業議會(議會)或香港特別行政區政府保留向旅行代理商及其提交的申請中所涉及的服務供應商作資料審核的權利。The applicant understands that the Travel Industry Council of Hong Kong (“TIC”) or the HKSAR Government reserves the rights to validate information in the application submitted by the applicant with the travel agent and relevant service suppliers for audit purposes.

3. 旅行代理商已核實每個本地特色旅遊旅行團的導遊持有旅遊業監管局所發出的有效「導遊牌照」，或視作有效「導遊牌照」的原有導遊證並向有關導遊支付服務報酬。The travel agent has verified that the tourist guide for each Characteristic local tour holds a valid Tourist Guide Licence issued by the Travel Industry Authority or a pre-existing Tourist Guide Pass taken as a valid Tourist Guide Licence, and has paid service remuneration to the tourist guide concerned.

4. 申請人明白如旅行代理商以同一本地特色旅遊旅行團申請議會或香港特別行政區政府／半政府機構所提供的任何其他資助，將不符合申請本鼓勵計劃的資格。申請人及旅行代理商知悉如旅行代理商被發現作出上述行為，該旅行代理商會被取消所有本地遊項目資助的申請資格，議會亦不會處理該旅行代理商所有提交而未獲審批的申請。The applicant understands that the travel agent is not eligible to apply under the Characteristic Local Tourism Incentive Scheme ("CTIS") if the travel agent applies for funding support from the TIC or the HKSAR Government/semi-government organisations with the Characteristic local tour. The applicant and the travel agent are aware that if the travel agent is found to have taken the aforesaid act(s), the travel agent will no longer be considered eligible for applying for funding support under all local tour initiatives. The TIC will not process any other applications (not yet approved cases) submitted by the travel agent.

5. 本申請表格及與其一併提交的證明文件內的所有資料，均屬真實、有效及正確，並顯示截至提交相關申請表格當日的真實情況。All the information provided in this Application Form and the supporting documents attached hereto, is true, valid and accurate, and reflects the up-to-date situation as of the date of submission of the relevant Application Form and documents.

6. 與本申請表格一併提交的照片均屬真實，並於本地特色旅遊進行期間到訪所涉及的旅遊地點及活動時拍攝。The photographs submitted in connection with this Application Form are true and were taken during the conduct of the Characteristic local tour visiting the designated sites and participating in the tourist experience activity concerned.

7. 旅行代理商承諾本地特色旅遊的活動遵守申請指引中所列的保育守則。
   The travel agent undertakes to comply with the conservation code of practice as set out in the Guide to Application during the activity(ies) of Characteristic local tour.

8. 旅行代理商承諾提交不少於此本地遊旅行團5名或百分之十的團員(以較高者為準)所填寫的「遊客滿意度調查」問卷 。The travel agent undertakes to complete questionnaires of the post-tour satisfactory survey by no less than 5 participants or 10% of the tour participants (whichever is higher) has been submitted.

9. 與本申請表格及與其一併提交的照片及證明文件內提供的任何資料如被發現是虛假、無效或不準確，旅行代理商承諾於議會或香港特別行政區政府通知的指定時間內，向議會及香港特別行政區政府償還按本鼓勵計劃獲發的全部鼓勵金連同利息。If any of the information provided in this Application Form, the photograph(s) and supporting documents attached hereto, is/are found to be false, invalid or inaccurate, the travel agent undertakes to repay the TIC and the HKSAR Government all the cash incentives disbursed under the CTIS plus interest within the time specified by the TIC or the HKSAR Government.

10. 申請人及旅行代理商知悉，如旅行代理商提供虛假、無效或不準確的資料以支持本申請，或就本申請故意遺漏或隱瞞任何重要資料，議會及香港特別行政區政府有權向旅行代理商採取其認為適當的任何行動，包括終止發放鼓勵金和提出司法起訴。The applicant and the travel agent are aware that if the travel agent provides false, invalid or inaccurate information to support this application, or wilfully omits or conceals any material information in relation to this application, the TIC and the HKSAR Government reserve the right to take any action deemed to be appropriate against the travel agent, including the termination of disbursement of cash incentives and filing of judicial action.

11. 申請人及旅行代理商明白提供虛假、無效或不準確資料，以及故意遺漏或隱瞞任何重要資料的後果，並承諾如上述資料日後有任何更改，會立即以書面儘快通知議會。The applicant and the travel agent understands the consequences of providing false, invalid or inaccurate information and of wilfully omitting or concealing any material information and promises to notify the TIC in writing as soon as possible of any changes to the above information in the future.

12. 申請人及旅行代理商承諾全面配合議會對旅行代理商申請的審核，包括按其要求提交所有相關文件、檔案、紀錄等。The applicant and the travel agent undertake to fully co-operate with the TIC for the purpose of assessing the travel agent’s applications by providing, upon its request, all relevant documents, files, records, etc.

13. 旅行代理商須就議會及香港特別行政區政府因本申請或相關事項而蒙受或招致的任何或一切損失、債務及申索作出全部彌償。The travel agent shall fully indemnify the TIC and the HKSAR Government for any or all of the losses, debts and claims suffered or incurred as a result of this application or related issues.

聲明乙部 Declaration (Part B)

1. 申請人明白及確定(1)本地遊旅行團接待入境旅客，該等旅客必須在香港停留至少一晚；(2) 該行程不可包括在旅行社安排下光顧商店購物，否則將不符合申請本鼓勵計劃的資格。The applicant understands and confirms that (1) the concerned tour group receives inbound tourists, such tourists must spend at least one night in Hong Kong; (2) the concerned tour itinerary shall not include shopping arrangements made by the travel agent, otherwise the travel agent is not eligible to apply under the Characteristic Local Tourism Incentive Scheme (“CTIS”).
   
   
2. 申請人及旅行代理商知悉如旅行代理商被發現違反上述安排，該旅行代理商會被取消所有本地遊項目資助的申請資格，議會亦不會處理該旅行代理商所有提交而未獲審批的申請。The applicant and the travel agent are aware that if the travel agent is found to be in breach of the aforesaid arrangement(s), the travel agent will no longer be considered eligible for applying for the funding support under all local tour initiatives. The TIC will not process any other applications (not yet approved cases) submitted by the travel agent.

提交此申請表後，系統會按照申請者所提供的電郵地址發出一封載有「 申請號碼」 的電郵以確認收到有關申請，申請人可以利用此申請號碼作為日後查詢及聯絡時使用。如超過了 24小時後仍未有收到相關電郵，請與議會聯絡。Upon submission of this e-form, a confirmation email with the "application ID" will be sent to the email address provided by the applicant. If you do not receive the email after 24 hours, please contact TIC.
